How to refer

We take it from there
We contact the patient, complete the free screening, verify coverage, schedule the home visit and send you the visit note.
Response times
- Referral acknowledged
- Same business day in most cases, and within one business day as standard.
- Patient contacted for screening
- Typically within one to two business days of referral.
- Home visit scheduled
- By appointment, seven days a week, including evenings and weekends.
- Visit note returned
- On request, to the referring provider and the patient's primary care provider.
- Facility after-hours consultation
- Available to partner senior living communities for fall management and acute musculoskeletal concerns.
Appropriate referrals
- Osteoarthritis of the knee, hip, shoulder or ankle needing injection therapy.
- Patients for whom travel to a clinic is the barrier to treatment — mobility limited, homebound, no transport, or a facility resident.
- Sacroiliac joint pain, trigger finger, lateral or medial epicondylitis.
- Patients with a scheduled total knee replacement who are candidates for pre-surgical iovera° cryoneurolysis.*
- Patients scheduled for total shoulder replacement, reverse total shoulder replacement or rotator cuff repair who are candidates for pre-surgical iovera° cryoneurolysis — a large share of our pre-surgical volume.
- Non-emergent, non-surgical fractures needing management and follow-up. We examine the patient, order or coordinate imaging on site, manage the course, and refer on if operative care becomes appropriate.
- Patients at repeated risk of falls, particularly facility residents. Fall prevention is a focus of the practice: we assess gait, strength, joint range of motion, treatable pain and whether the patient's walker or cane is the right device and correctly fitted. A more formal program including pharmacist-led medication review is in development.
- Persistent pain after knee arthroplasty.
- Chronic nerve pain of the hip, knee, shoulder, foot or ankle — excluding diabetic peripheral neuropathy.
- Phantom limb pain; Morton's neuroma.
Not appropriate for us
- Acute trauma, an open or grossly deformed fracture, a suspected hip fracture, a dislocation, or any emergency — send to emergency care. Non-emergent, non-surgical fractures are appropriate for us; see above.
- Suspected septic joint or systemic infection.
- Surgical candidates needing an operative opinion — we do not perform surgery.
- Long-term opioid management or controlled substance prescribing.
- Advanced imaging. We perform point-of-care diagnostic ultrasound and we order plain radiography and act on the radiologist's report, but we do not provide MRI or CT.
- iovera° for diabetic peripheral neuropathy.
What the patient is told
Every referred patient gets the free telephone screening first. If we are not the right service for them, we tell them and we tell you, rather than scheduling a visit that will not help.
